The contractor shall provide community-based lead screening, testing, outreach, education, training, and follow-up support services for children ages 6 months through 6 years, pregnant people, and families facing barriers to care. Services shall include point-of-care capillary testing when appropriate and aligned with DOH/HI-CLPPP guidance; family education and outreach; testing events in community-based settings; coordination with DOH Public Health Nurses for confirmatory testing and follow-up; training and technical assistance for FQHCs, pediatric offices, and community clinics; and support for DOH-led investigation and response efforts related to elevated blood lead levels. Subrecipient shall document and report project activities, referrals, training, outcomes, barriers, and recommendations in accordance with DOH/HI-CLPPP requirements. Outcome 1: Decrease disparities in blood lead levels by socioeconomic status and improve blood lead testing and reporting rates for children less than 6 years of age at risk for lead exposure. Outcome 2: Decrease number of children living in environments at high risk of lead exposure and improve rates of children less than 6 years of age with elevated blood lead levels linked to recommended services.
