This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Small Businesses interested and capable of providing the services described below. The priority is to find two or more Service-Disabled Veteran Owned Small Businesses (SDVOSB), Veteran-Owned Small Businesses (VOSB). Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number and (h) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below.
CONTRACT REQUIREMENTS
Contract Title: Medical Gas Testing and Inspection Place of Performance: VASDHS, 3350 La Jolla Village Drive, San Diego CA 92161 Background: The VASDHS at 3350 La Jolla Village Drive, San Diego CA, CA 92161 is part of the Desert Pacific Healthcare Network (VISN 22), and is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the art technology as well as education and research. Scope: To test the VA San Diego Medical Gas System for a condition of compliance with NFPA 99 Healthcare Facilities Code. Specific Tasks: The basis of work for this SOW is to conduct inspections of VA San Diego s Medical Gas System. The inspection must be conducted by certified medical gas inspectors. Inspectors must verify all medical gas systems including but not limited to patient terminal outlets/inlets, alarm systems (master, area, and local), shut off valves (zones, isolation, service, source, and main), and source equipment. Report all findings. Any previous changes made to the medical gas system shall be re-certified as directed by NFPA 99. A. Scope: Task 1: Test all positive and negative pressure medical gases accordance with the most current NFPA99 and ASSE 6000 requirements. Conduct the following tests/inspections: 1. Standing Pressure Test NFPA 5.1.12.4.2 2. Cross-connection Test NFPA 5.1.12.4.3 3. Valve Test NFPA 5.1.12.4.4 4. Alarms NFPA 5.1.12.4.5 5. Piping Purge Test NFPA 5.1.12.4.6 6. Piping particulates Test NFPA 5.1.12.4.7 7. Piping Purity Test NFPA 5.1.12.4.8 8. Medical Air Purity Test NFPA 5.1.12 9. Monthly Area Alarm: Activate all audible and visual signals using test button. Test master alarm functions for audible and visual signals. Vacuum area alarm panel with test buttons: Activate the audible and visual alarm signals Area alarm panel gauges/pressure readouts: Check line pressure System main line low- and high-pressure alarms: Test audible and visual signals at the master alarm panels. Manifold reserve in use alarm: Test the audible and visual signals. Reserve Supply Low warning signals: Test audible and visual signals. De point and CO alarm: Test audible and visual signals. Vacuum main line low pressure alarm: Test audible and visual signals at the master alarm panel(s). Medical air compressors and vacuum pumps: Check for proper operation and preventive maintenance per manufacturers specifications. Pressure check across filters. Oxygen concentrator units: Check for proper operation and preventive maintenance per manufacturers specifications.
10. Quarterly Intake for medical air compressors: Check intake location and filter for trash and debris. Source evaluation on medical air and vacuum systems: Function of automatic alternating controls Correct pressure/vacuum switch Frequency of pump starts and duration of runs Cut in/out pressure/vacuum
11. Semi-Annually Manifold system: Leak check (i.e. valves, pigtails, and regulators). Visually inspect cylinder pigtails for physical damage.
12. Annually Check all outlets in critical care areas: Flow, pressure/vacuum, damage and wear (e.g. ICU, CCU, NICU, Surgery, Recovery, ER). Bulk liquid systems: Test the operation of the reserve and activation of the reserve-in-use signals. Cryogenic fluid central supply systems: Check proper level activating switch. Manifold reserve-in-use alarm: Test proper function of switch Manifold: Test operation of the changeover for the secondary supply Pressure gauges: Assure gauges are mechanically functional Air compressor: Performance testing Dew point and CO Monitors: Calibrate and test operation Maintenance and periodic testing of the cryogenic fluid supply system in conjunction with the supplier of cryogenic fluid central supply.
13. Periodically Shut-off valves: External leakage test Station outlets and inlets: Check for leakage and flow Calibration of pressure gauges Inspection and test of EOSC for proper operation.
Task 2: Evaluation of Findings and Recommendations a. Prepare written Statement of Findings and Recommendations. Task 3: Repair findings a. Provide an additional 200 hours of labor and repairs to keep the facility medical gas system operating in optimum conditions during the contract. These 200 hours of labor are for repair services, not included in the labor to conduct the required NFPA inspections/testing. It will be used for additional testing of the system on all faulty, or damaged devices/equipment identified during inspections and emergency malfunctions. The 200 hours is the normal business hour rate and holiday/emergency/after-hours rates will be adjusted and calculated from the normal working hours total dollar value. Labor will be prorated based on normal working hour to emergency and holiday response hours and invoiced immediately after additional work is completed. Task 4: Service calls a. Provide service calls on-site, for repair service, as needed throughout the contract life. When the medical gas system requires repair or malfunctions occur, the contractor shall repair and provide parts, not to exceed $2,000.00 per current annum of contract. This includes the cost, of parts at cost to vender along with shipping and handling. Part will be invoiced separately and at the time of install or acceptance. If more than $2,000 is required, a contract modification will be required. Contractor will be required to monitor annual expenditures and advise COR prior to going over the allotted $2,000. Deliverables: Contractor shall provide the following: 1. Contractor shall provide an electronic format report and a paper copy of the Medical Gas System. Report shall cover the verifier s certifications and the test results. Results shall define the scope of the work performed and shall include details of actions and methods used to perform inspection and recommendations. During data collection, Contractor shall report discrepancies in existing system. Performance Monitoring: VASDHS project COR shall monitor Contractor work based on the following criteria but not limited to: 1. Knowledge of Medial Gas System inspection procedures, and NFPA 99 requirements. 2. Ability to perform work and provide reports at the specified dates. 3. Proven exemplary work history in medical facility systems. 4. Contractor certification to perform work in Medical Gas Systems. 5. Familiarity with VA procedures and specifically, the VASDHS facility. 6. Employee qualifications and experience in performing required work. 7. Past similar work on-site or within other VA facilities or Federal entities of comparable size. Safety Precautions 1. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in NFPA 99, Centers for Disease Control and Prevention (CDC), Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, CFR 1910.132(d)(1)(iii), 29CFR1910.137 2. In the performance of this contract, Contractor shall take such safety precautions as necessary while operating in construction area. Contractor shall provide all necessary tools, equipment, labels, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Other Pertinent Information or Special Considerations 1. Contractor employees will notify the VA Representative before entering a designated permit required confined space so that appropriate precautions may be taken. In addition, there are special precautions to be taken when working on 2S Mental Health unit or other locations of this facility. If required, Contractor shall keep tools and materials under their control at all times. The facility will be functional during contractor operation. Contractor shall therefore schedule the work accordingly and coordinate operation with the facility construction safety manager and project s COR. Several inspections will be conducted in locations that may require additional personal protective equipment. Contractors will notify the VA representative at any time the contractor is on VA grounds. 2. VASDHS La Jolla facility is Federal property. Contractor shall coordinate with the facility Engineering Service if warranted for access in restricted spaces or any required support in the performance of this work. The designated COR shall coordinate the request. 3. If work will be conducted in areas identified as containing asbestos materials. Contractor shall comply with the public laws and statues including all changes and amendments of Federal, State, and local environmental statutes and regulations governing Asbestos containing materials-ACM, to include: 29 CFR 1926.1101 Asbestos Regulations for the Construction Industry. 29 CFR 1910.134 Respiratory Protection Standard. EPA, Asbestos Hazard Emergency Response Act (AHERA) protocols. Guidance for Controlling Asbestos-Containing Materials in Buildings, EPA 560/5-85-024.
Acceptance Criteria: The COR can perform inspection of the work performed without the presence of the contractor and determine whether the work performed is within standards expressed in this scope of work. It is VA prerogative to hire an independent qualified third party to check the validity of the work conducted. CONTRACTOR QUALIFICATIONS & OTHER RESPONSIBLITES: No subcontracts are allowed. Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. The Government reserves the right to accept or reject Contractor s employee for the rendering of services. Complaints concerning Contract Personnel s performance or conduct will be dealt with by the Contractor and COR with the final decision being made by the Contracting Officer.
SAFETY REQUIREMENTS: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. The VAMC San Diego building 1, main building, contains asbestos. Any worker entering these asbestos containing areas must have an asbestos certification of no less than 16 hours and a valid fit test certification. Certifications must be presented to the VA project manager before any work in an asbestos area may begin.
CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof.
VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court.
PARKING POLICY: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. IDENTIFICATION BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. SMOKING POLICY: Smoking is not permitted within the VA Healthcare System facilities campus. NORMAL WORKING HOURS: Normal hours of operation are Monday through Friday from 8:00 am to 4:30 pm Pacific time. All work is expected to be performed generally during normal hours of coverage unless requested or approved by the COR or his/her designee. Before commencement of work, the Contractor shall confer with the VA COR and Contracting Office to agree on a sequence of procedures; means of access to premises and building. All work performed, to include delivery of materials and equipment, shall be made with minimal interference to Government operations and personnel. Work performed outside the normal hours of coverage must be approved by the COR or his/her designee.
OVERTIME AND NATIONAL HOLIDAYS: Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.
