EVALUATION MATRIX Factor Name|Max Points Price of E-Rate Eligible Equipment|35 Equivalency of Equipment|33 Experience with the District|20 Local/In-State Vendor|12 TOTAL Points|100 The highest score will go to the lowest price offered for the equipment specified. Those vendors matching the requested equipment will be given the highest score. The committee will determine the equivalency of the items listed. Those vendors that have had prior working experience with the DSD will be awarded more points. Those vendors with a local presence, locally within the Salt Lake Valley area, will be awarded more points.
The awarded pricing will be available through 1 March 2027. Some of the E-Rate eligible equipment may be purchased as early as 1 April 2026, but the majority will be purchased after 1 July 2026. The contract will be contingent upon a positive Funding Commitment Decision Letter (FCDL) awarding E-Rate discounts. Purchase orders will be sent upon receipt of an E-Rate FCDL. Any such order will be made as early as 1 April 2026, with the majority being made after 1 July 2026. If a positive FCDL is issued, then DSD may purchase part of or up to all of what is listed in the awarded contract at separate times during the contract period. If a positive FCDL is NOT issued, then DSD may opt out of the contract and not complete the purchase as listed or may purchase part of what is listed in the awarded contract. The contract must follow the terms and conditions as outlined in the attached document to the Form 470 with filename, "DSD Contract Terms and Conditions Rev June 2023.pdf".
