2026 Form 470 Lone Tree CSD Internet Service

Lone Tree Community School District is seeking a bundled Internet Access service (IA and associated transport). This service is to be delivered to the hub of the districts network at 303 Devoe St, Lone Tree, IA 52755. District seeks bids for Internet at the following capacities: Minimum of 1 GBPS and maximum of 5 GBPS. District also requests bid options for one-year, three-year, and five-year contracts. Interested vendors should also provide the following information in their bids: a) Indicate the type of connectivity (e.g. fiber, coaxial, microwave, etc.); b) Indicate whether E-Rate C1 eligible bundled firewall is provided; c) Identify any E-Rate ineligible charges (such as DDoS); d) Indicate the cost of IPv4 Public IP addresses and if these can be provided. If there is a fee, indicate the dollar amount separately; e) Bidders must be willing to provide turn-up testing and initial connection no less than ten (10) business days prior to the start of service; f) Indicate the actual dollar amount of any applicable taxes and fees (not just a statement that taxes and fees are separate), g) Bidders must be willing to provide E-Rate discounted billing and to include this statement in the proposal. Robo-bids and bids sent as SPAM without regard to products and services listed on this Form 470 and/or the RFP will be disqualified and will not be evaluated. The District reserves the right to cancel this request for bids and to reject all related proposals.

State/local procurement requirements: By submitting a bid on the requested services herein, the vendor certifies: its proposed prices are consistent with the FCCs Lowest Corresponding Price (LCP) requirements and that its equipment and services are compliant with the FCCs recent Order (FCC 19-121) prohibiting the sale, provision, maintenance, modification, or other support of equipment or services provided or manufactured by Huawei, ZTE, Hytera, Hangzhou, Dahua, Pacific Network Corp. and its wholly-owned subsidiary ComNet (USA) LLC and China Unicom (Americas) Operations Limited or any other covered company deemed a national security threat.
