The suggested contractor is W. L. Gore, 1500 North 4th Street, Flagstaff, AZ 86004-2400 1 each Gore DualMesh Plus Implants, 15cm x 19cm, Catalog Number: 1DLMCP04. 1 each Gore DualMesh Plus Implants, 18cm x 24cm, Catalog Number: 1DLMCP06. 1 each Gore DualMesh Plus Implants, 20cm x 30cm, Catalog Number: 1DLMCP07.
