This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This solicitation is a Total Service-Disabled Veteran-Owned Small Business set aside, and is a Request for Quote (RFQ), number VA261-17-Q-0819. This announcement constitutes the only solicitation; a quote is requested, and a written solicitation will not be issued. The North American Industry Classification System (NAICS) code is 339113 Surgical Appliance and Supplies Manufacturing, and the size standard is 750 employees. The San Francisco VA Medical Center has a requirement for bed frames, mattresses, low air loss pumps, and maintenance that includes inspections, parts, and labor for one (1) year. Quoters should thoroughly review the Price Cost Schedule of Supplies and Services and Statement of Work (SOW), and be familiar with the requirements of the solicitation prior to submitting a quote in order to be fully aware of the scope of supplies and services required. Failure to do so will not relieve the successful quoter from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Federal Government. The Government shall not reimburse any cost not incorporated into the quoter s price, except as otherwise set forth herein. Respond to the requirements of this RFQ by completing the blanks in the Price Cost Schedule of Supplies and Services and by including a completed copy of Federal Acquisition Regulation (FAR) 52.212-3 Offeror Representations and Certifications Commercial Items (DEC 2016) with quotes. Both are attached to this RFQ. Also, vendors shall include local stock numbers with quotes. Quotes and FAR 52.212-3 Offeror Representations and Certifications Commercial Items (DEC 2016) should be emailed to contact@example.gov. Please ensure that the subject line of all emails include VA262-17-Q-0819. Quotes are due by 4 p.m. Eastern Standard Time (EST) on August 18, 2017. Technical inquiries shall be emailed to contact@example.gov. Technical inquiries will be accepted through 1 p.m. EST, on August 4, 2017. For technical inquiries, please ensure that the subject line of all emails include VA261-17-Q-0819. Telephone requests for information or questions will NOT be accepted. No hard copies of this RFQ will be provided. CLAUSES, PROVISIONS, AND ADDITIONAL INSTRUCTIONS 1. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. 2. FAR 52.212-1 Instructions to Offerors-Commercial Items (OCT 2016) and FAR 52.212-4 Contract Terms and Conditions Commercial Items (MAY 2015) apply to this acquisition and no addenda are attached. 3. FAR 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders Commercial Items (JAN 2017), applies to this acquisition. The clause is attached to this solicitation. 4. Veteran Affairs Acquisition Regulations (VAAR) clause 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (DEC 2009) applies to this acquisition. 5. The procurement of these commercial supplies and/or services is in accordance with FAR part 12 Acquisition of Commercial Items, FAR part 13 Simplified Acquisition Procedures, and the VAAR, as supplemented with additional information in this notice. 7. BASIS FOR AWARD: Subject to the provisions contained herein, award shall be made to the quoter deemed responsible in accordance with the FAR and who conforms to the solicitation requirements; and whose quote, judged by an overall assessment of the quoter s qualifications specified in this solicitation, meets the technically acceptable standard for the non-price factors and provides the lowest evaluated price. The VA intends to issue a firm fixed-price purchase order for commercial supplies and/or services. Any quote(s) submitted for any name brand items must be from authorized distributor(s), reseller(s), partner(s), or manufacturer(s), or quote will be rejected as nonresponsive.
