(ii) The solicitation number is VA249-16-Q-0648 and the solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86-1, March 16, 2016. (iv) This requirement is unrestricted. (v) CLIN0001 - Twelve (12) Boxes of Ten (10) each Vascular Closure Devices, Collagen Plug (Brand Name or Equal to Angio-Seal). (vi) See the attached 1449 CLINS for more information of the description of CLIN0001 above. (vii) The date of delivery is ARO+10. The place of delivery is 1030 Jefferson Avenue Attn: 111, Tiensha Salter, Memphis, Tennessee 38104. FOB is Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition.
