This amendment is issued to correct the original response due date from: June 6, 2004 to: May 6, 2004. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. FHQ04Q00075 is being issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. The National American Industry Classification System codes are 325620, 325611 with a corresponding size standards of 500; this requirement is set-aside for Small Business only. The U.S. Department of the Treasury, Financial Management Service (FMS), 401 14th Street, SW, Washington, DC, 20227, intends to award a purchase order utilizing simplified acquisition procedures and provisions and clauses for Commercial Items for Air freshener units, Auto flush units, Solar air freshener units, Direct feed sanitizer units, Hand-free sanitary waste disposal units (requires special handling for disposal of sanitary waste products), Sanitary wipes, Foam soap dispenser units. These items will be required at the following addresses in the required amount: Financial Management Services, Liberty Center Building, 401 14th Street, SW, Washington, DC, (22 - Air freshener units, 14 -Auto flush units, 2 - Solar Air freshener units, 76 - Direct feed sanitizer units, 33 - Hands-free sanitary waste disposal units). The government will award a commercial item purchase order using simplified acquisition procedures to the responsible offeror whose offer is most advantageous to the government. The offers will be evaluated on the basis of product description as stated above, past performance, and price. Response must be in writing by above-mentioned date. Responses must be in writing and must be received within ten calendar days of publication of this notice. Responses must include the Representations and Certifications and the VETS 100 Compliance Report.
