ADDENDUM #2 RFP 2026-002 Veteran-Directed Care Assessment and Options Counseling Services

The Santee-Lynches Regional Council of Governments, through the Santee-Lynches Area Agency on Aging, is soliciting proposals for Veteran-Directed Care (VDC) assessment and options counseling services under RFP 2026-002. The selected contractor will provide intake, enrollment support, and participant-centered services to help eligible Veterans remain safely in their homes and communities. The scope of work requires the ability to serve referrals from VA Dorn across 36 counties in South Carolina and 3 counties in North Carolina. Key performance requirements include scheduling initial meetings within seven days of a referral and conducting minimum quarterly in-home visits supplemented by monthly virtual or telephonic contact. The contract is for a one-year term starting November 16, 2026, with an option to renew for up to a two-year extension. Proposals are evaluated on a 100-point scale, with the agency reserving the right to negotiate or award contracts to one or more proposers. Payment is issued via check on a 90-day invoicing cycle, contingent upon confirmation from Bay Aging and the completion of all required documentation, including wet signatures. Contractors must comply with HIPAA and 45 C.F.R. requirements, maintain necessary insurance, and certify they are not excluded from federal health care programs. Proposals must be submitted in person, by mail, or via courier by October 30, 2026, consisting of one original and three copies.
