B -- OBSERVING FITNESS

PURPOSE: Contractor will provide consultant services and technical support to Navajo Coordinated School Health, the 40 Navajo Area - wide schools and SOFIT (System for Observing Fitness Instruction Time) field observers. Contractor will analyze data, compile, and submit final executive and extended written reports for the 2005-2006 & 2006-2007 school year. System for SOFIT is one of the evaluation methods for Navajo Coordinated School Health. Contractor will collect SOFIT data from Navajo Area Indian Health Service Unit School Health programs involving 40 elementary schools, Kindergarten through sixth grade accoss the Navajo Nation. Contractor will analyze SOFT data collected from September 2005 through April 2007 from 40 schools. The data will be collected by the Health Promotion Program SOFT observers and forwarded to the contractor. Contractor will enter and track monthly data. The contractor will compile a report, at the conclusion of the school year. This final report will contain pertinent information like number of schools involved, number of observations, comparison or differences from last year baseline summary and any recommendations to improve on physical educaton programs in the schools. Contractor will be available for any technical assistance from the school or trained observers via phone or e-mail, Contractor will provide a summary report to Navajo Area School Health Coordinator at Shiprock Service Unit. Upon award of contract contractor will initiate project and carry it out over the following year. The project will be within the time of the purchase order issued through September 30, 2007. No government property will be involved in carrying out this contract. Quality Assurance Provision: These will include inspection of preliminary and final materials delivered to Navajo School Health Coordinator, Shiprock Service unit and review of interim reports by Community Health Service staff. Progress meeting will be place in person, though email or by phone at least monthly between contractor and navajo Area School Health Coordinator during the performance period of the contract. Contractor will provide services that are in support of the Health Promotion Department Program Plan and OBjective in support of Wellness Programs.
