Issuing Procurement Unit: State of Utah Division of Purchasing Conducting Procurement Unit: Utah Department of Health and Human Services  
The purpose of this Request for Proposal (RFP) is to enter into a contract with a qualified Offeror for the rental of oxygen concentrators, associated equipment, supplies, and maintenance for Utah Medicaid Members.
The State will award a contract to a single Offeror who must provide Concentrators and associated equipment for all eligible, enrolled Medicaid Members (as defined in Attachment B: Scope of work, Section 1.1, Definitions) statewide.
Furthermore, the Offeror must render any other equipment or supplies ordered by a treating Practitioner to meet the respiratory needs of the Member when a Concentrator has been ordered. These items are separately reportable by the Offeror with reimbursement based on the Medicaid fee schedule (e.g. ventilators, CPAP, BiPAP, pulse oximeters, and sufficient backup and portable oxygen to meet the member’s medical needs).
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code.
Length of Contract: Five Years
Vendor must guarantee its pricing for One year.
