BUENA PARK SCHOOL DISTRICT hereinafter the District is seeking to contract with a Company hereinafter Proposer to assist the District in providing internal connections material/hardware and or services for locations described in the Scope of Work of the RFP. For a complete description of equipment and services requested please refer to District issued RFP at www.bpsd.us/requests and also the USAC EPC website https://portal.usac.org/suite/. Any addendum will be posted & available on the District website as well as the EPC website. Please Follow the Form 470 #260004878 to receive all EPC updates pertaining to the Form 470. Questions regarding the requested services in which Buena Park SD is seeking under Form 470 and District issued RFP must be emailed to mmagboo@bpsd.us no later than November 17, 2025 by 2:00pm PST and all answers to questions will be posted on the district website as well as USACs EPC on November 20, 2025. Questions emailed should reference Buena Park SD Form 470 #260004878 and clearly identify Buena Park SD_26_27_Cat2_RFP 252608 BPSD E-Rate 1172025 in the subject line. Proposal Instructions: Proposals will ONLY be received on or prior to 1:00 PM on December 18, 2025 after which time the proposals will be opened and reviewed in a closed session by the evaluation committee. All proposals shall be provided in hard copy and in Digital PDF format as well. Hard copies of the bid and a digital copy of the bid on a USB pen drive shall be submitted no later than 1:00 pm PST, Thursday, December 18, 2025, at the District Office, located at 6885 Orangethorpe Ave., Buena Park, CA 90620, in care of the Director, Information Technology. Proposals must be received no later than the time and date designated above. Proposals received later than the designated time and date will not be accepted. Facsimile (FAX) copies of the proposal will not be accepted. All responsive proposals will be reviewed and evaluated in a closed session by the District in order to determine which proposer best meets the Districts needs for this project. The criteria by which the District shall evaluate proposals are set forth in the RFP NOTE: All quantities are estimates as solutions proposed by vendors must meet the minimum specifications required. Please refer to the RFP for full specifications and requirements. IMPORTANT: The proposed solution price must include a complete bill of materials, applicable sales tax, and applicable shipping. Please note that while the District is seeking a one-time capital investment in infrastructure, if a service provider's solution contains some elements of the service types "Basic Maintenance of Internal Connections" (BMIC) and/or "Managed Internal Broadband Services" (MIBS) as defined by the manufacturer and the approved Eligible Services list, the District will evaluate these proposals as long as they meet the RFP requirements. The District is not soliciting separate bids for BMIC or MIBS as stand-alone solutions.
State/local procurement requirements: PLEASE SEE RFP. Applicant may consider multi-year and/or contracts with voluntary extensions. Any voluntary contract extensions must be identified within the awarded contract and must be of a specified number and duration. Contracts with automatic or evergreen contract extension terms will not be considered. Applicant may consider contracts with flexible terms to allow for growth/reduction in services to accommodate an increase/decrease in the number of sites, users, and/or bandwidth. Service Providers submitting proposals in response to this FCC Form 470 must be in compliance with the rules and orders governed by the Federal Communications Commission. Failure to be in compliance and remain in compliance may result in the denial of discount funding, and/or cancellation of funding commitments and/or contracts, and/or could result in civil or criminal prosecution by law enforcement authorities. Service Providers submitting proposals must do so in good faith of compliance with the Lowest Corresponding Price (LCP) Rule (see 47 CFR Part 54 Section 54.500(f)). Service Providers submitting proposals must be in full compliance with USACs Free Services Advisory https://www.usac.org/e-rate/applicant-process/competitive-bidding/free-services-advisory/. Any offering of free services must be clearly identified in service providers bids. Service Provider invoicing to USAC must be completed within 120 days from the last day of service. Should the Service Provider fail to invoice USAC timely, Applicant will only be responsible for paying its non-discounted share. The Service Provider acknowledges that all pricing and technology infrastructure information in its contract shall be considered as public and non-confidential pursuant to CFR Part 54 Section 504 (2)(i)(ii). Please note that while the District is seeking a one-time capital investment in infrastructure, if a service provider's solution contains some elements of the service types "Basic Maintenance of Internal Connections" (BMIC) and/or "Managed Internal Broadband Services" (MIBS) as defined by the manufacturer and the 2025 Eligible Services list, the District will evaluate these proposals as long as they meet the RFP requirements.
