TEST REPORTS AND COMPONENT CERTIFICATIONS IN ACCORDANCEWITH DLA TROOP SUPPORT CLOTHING & TEXTILES ADDITIONAL QUALITY ASSURANCE REQUIREMENTS, Revision 1 dated 1 August 2021 (supersedingDPSCM-4155.3 dated 3 November 1997) ARE REQUIRED WITH ALL CONTRACT COMPONENT LOTS. . In lieu of the abovenamed requirement, if thetotal contract award does NOT exceed $100,000, the contract awardee may submit a Certificate of Compliance (CoC) per 3.1.3 of theDLA Troop Support Clothing and Textiles (C&T) Additional Quality Assurance Requirements (AQAR), Revision 1 dated August2021.
Please note, however, use of the CoC in lieu of submitting the certified test reports, is subject to approval by theContracting Officer identified on the solicitation, purchase order, and/or the contract. . SOURCES OF COMPONENT MATERIALS SHALL BE IDENTIFIED, INCLUDING APPLICABLE SPECIFICATIONS, AND SHALL NOT BE CHANGED THROUGHOUT THE LIFE OF THE CONTRACT WITHOUT REQUEST ANDAPPROVAL BY THE DLA TROOP SUPPORT CONTRACTING OFFICER THROUGH MODIFICATION TO THE CONTRACT.

. GOVERNMENT LABORATORY TESTING OF COMPONENTS FOR ACCEPTANCE PURPOSE
FOR PHYSICAL (NON-SHADE) TESTING: Only applicableif the total contract award exceeds $100,000
In lieu of the abovenamed requirement, if the total contract award does NOT exceed$100,000, the contract awardee may submit a Certificate of Compliance (CoC) per 3.1.3 of the DLA Troop Support Clothing and Textiles (C&T) Additional Quality Assurance Requirements (AQAR), Revision 1 dated August 2021. . THE VENDOR SHALL NOTIFY THE DLA ProductTest Center -Analytical FOURTEEN (14) CALENDAR DAYS TO THIRTY (30) CALENDAR DAYS IN ADVANCE OF WHEN LOTS ARE TO BE PRESENTED FORSAMPLING AT THE ADDRESS BELOW.
THE CONTRACTOR'S APPROVED QUALIFIED LABORATORY WILL BE REQUIRED TO FORWARD A COMPLETED TEST REPORTFOR EACH LOT OF COMPONENTS TO THE ADDRESS BELOW. THE SUPPLIER'S SOURCE SAMPLING REPRESENTATIVE WILL PROVIDE THE COMPLETED DD FORM1222 dated March 2021 TO THE LABORATORY TO BE SUBMITTED WITH THE COMPLETED TEST REPORT. TEST REPORTS SHALL NOT BE OLDER THAN NINETY(90) CALENDAR DAYS.
