A complete written service and installation report is required, detailing the date of service, technician names, materials used, model and serial numbers, VA barcode (EE) numbers, replaced parts, and the contract obligation number. Additionally, manufacturer warranty documentation, Material Safety Data Sheets (MSDS), and disposal documentation must be submitted. The solicitation is exclusively set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) certified in the SBA database, and only SDVOSBs listed in the official SBA certification system are eligible for award. Offerors must not submit SBA verification letters. The prime contractor must comply with limitations on subcontracting, ensuring no more than 50% of the contract value is paid to non-certified entities. Proposals must be submitted in PDF format via email to crystal.carabajal@va.gov by 3:00 p.m. CDT on July 20, 2026, with the solicitation number and “Proposal Submission” in the subject line. The purpose of this project is to remove and replace the existing carpet in the main Chapel located in Building 99 at the Central Texas Veterans Health Care System (CTVHCS) located at 1901 Veterans Memorial Dr. Temple, Texas 76504. The Contractor must furnish all labor, materials, equipment, supervision, and disposal necessary to perform all demolition, removal, replacement, and installation work required under this Statement of Work and applicable specifications. The Contractor is responsible for the proper disposal of all debris, removed carpet, padding, tack strips, adhesives, packaging, and any other materials generated during demolition and installation.
