In accordance with FAR 10.001(a)(2), the Social Security Administration (SSA) is conducting market research to determine whether GSA Schedule vendors are capable and interested in meeting potential SSA requirements for physicians. SSA is seeking sources with GSA Schedules to acquire services of Family Practice or General Internal Medicine physicians as defined in the Statement of Work (Attachment Number 1) to provide clinical and advisory health services as the medical officers in the Employee Health Center (EHC) located at specified sites throughout the country. The hours required each week at each location vary and some locations require only 10-15 hours per week. SSA Sites: - 1 1. Office of Disability Adjudication and Review (ODAR) Headquarters- EHC One Skyline Tower Suite 101 5107 Leesburg Pike Falls Church, VA 22041 Contractor shall provide the services of a physician four (4) hours each day from 9:15 a.m. to 1:15 p.m., two (2) days per week on Wednesday and Thursday. - 2. SSA- EHC Southeastern Program Service Center 1200 Rev. Abraham Woods, Jr. Blvd Birmingham, AL 35285 The Contractor shall provide the services of a physician ten (10) hours weekly from 11:00 a.m. - 4:00 p.m. on Tuesdays and 7:00 a.m.-12:00 p.m. on Thursdays. - 3. 3. SSA - EHC 6401 Security Blvd 271 Altmeyer Building Woodlawn, MD 21235 The Contractor shall provide the services of a physician forty (40) hours a week, 8 hours per day plus 30 minutes of unpaid lunch, between the hours of 7:00 a.m. and 6:00 p.m. - 4. 4. SSA- EHC Northeastern Program Service Center Joseph Addabbo Building 155-10 Jamaica Ave Jamaica, NY 11432 The Contractor shall provide the services of a physician fifteen (15) hours weekly from 8:00 a.m. to 11:00 a.m. Monday through Friday. A general statement of work is provided as Attachment Number 1 to this notice. Please submit expressions of interest in your own format to include documentation of your capabilities in sufficient detail for the Government to determine that your company possesses the necessary expertise and experience to compete for this acquisition. Please provide your GSA contract number and indicate which sites your company is interested in providing physicians in accordance with your GSA contract rates.
