Full Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 13 Simplified Acquisition Procedures and (FAR) Subpart 12.6 entitled "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included in this notice. This announcement constitutes the only solicitation proposals are being requested and a written solicitation will not be issued. This solicitation, N00014-12-R-0008, is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60 effective 26 July 2012 and DFARS Publication Notice 20120724. The associated North American Industry classification System (NAICS) code is 713940 and small business size standard is $7.0M. The Office of Naval Research (ONR) has a requirement to provide fitness services through its Health Club Membership to ONR active duty military and federal government civilian employees. Offeror(s) must provide health club facilities in the Ballston Arlington, VA area, which are within ten (10) minute walking distance from ONR's offices located at 875 North Randolph Street, Arlington, VA. Facilities should provide (1) resistive weight training machines, (2) cardio-fitness equipment, (3) aerobics classes, (4) free-weights and related apparatus, and (5) locker rooms. Offeror(s) must also provide (1) a health risk assessment for each member, (2) a monthly report listing the members and the amount of monthly utilization by each member, and (3) liability insurance for all facilities used by ONR active duty military and federal government civilian employees. The government intends to award a Firm Fixed Price Purchase Order that represents the best value to the Government. The Order will include a twelve (12) month base period and four (4) twelve (12) month option periods. The performance period will not exceed sixty (60) months. The requirement details are stated in Attachment Number 1 of this announcement. The representations and certifications are provided as Attachment Number 2.
