The purpose of Amendment 0001 to RFQ FA500026Q0031 is to:


1. Add questions and answers to solicitation.


2. Add Asbestos Testing Results.


Please see the attached updated RFQ document and Asbestos Testing Results (with changes highlighted) for full details. All other terms and conditions remain unchanged.



The contractor shall provide all personnel, equipment, facilities, transportation, and materials necessary to perform a complete flooring replacement in Building 11540, Room 5. The work includes the demolition and off-site disposal of existing glued-down broadloom carpet and rubber wall base, comprehensive concrete subfloor preparation, and the installation of new commercial-grade Luxury Vinyl Planking (LVP) and rubber transitions. All work must be performed in strict accordance with the attached Performance Work Statement (PWS).

  • Site Visit: A highly encouraged site visit will be held on 8 May 2026 at 9:00 AM AKT. Attendees will meet at Bldg 11540, Rm 5, on Slammer Ave, JBER, AK 99506. Please RSVP to the Point of Contact.

  • Questions Due: All questions must be submitted via email no later than 12 May 2026 at 12:00 PM AKT.

  • Labor Requirements: This contract is subject to the Service Contract Act. The applicable Department of Labor Wage Determination for Anchorage Municipality, Alaska (WD 2015-5681) is attached to this solicitation.
