This contract specifies the procurement of two units of disposable adult defibrillator electrode pads designated by NSN 6515-01-613-1594 and manufacturer part number 9660-001 from Atlantic Diving Supply, Inc., intended for use with the AED G3 PRO system. Each pad is polarized, weighs 0.2 pounds, and is classified as a Type I (Code M) medical device with a strict non-extendable shelf life of 24 months. The items must be packaged in sealed commercial unit containers that prevent damage or breakage and packed in commercial shipping containers suitable for safe transport at the lowest cost to the designated delivery point. All packaging and marking must comply with Medical Marking Standard No. 1, which supersedes MIL-STD-129, and copies of the standard are available through DLA Troop Support. The product is regulated by the FDA, requiring referral confirmation via EBS before award. Delivery is required FOB destination within 84 days of contract issuance, with no tolerance for quantity variance. The shipment destination is the U.S. Medical Materiel Center in APO AP 96260-5479, with an additional freight shipping address in Korea.
