Provision of Care Navigators

Tees, Esk and Wear Valleys NHS Foundation Trust, with Synchronicity Care Ltd acting as the procurement coordinator, is seeking bids for the provision of Care Navigators under solicitation C486014. This initiative is part of a community mental health transformation programme designed to help service users, carers, and families independently access local community services through the support of multi-disciplinary teams and partner agencies. The contract is divided into four geographic lots: Lot 1 in Middlesbrough (2.4 FTE, estimated at 251,538 GBP), Lot 2 in Redcar (1 FTE, estimated at 104,808 GBP), Lot 3 in Darlington (1 FTE, estimated at 104,808 GBP), and Lot 4 in Stockton (2 FTE, estimated at 209,616 GBP). The total estimated base value is 670,770 GBP excluding VAT. The initial performance period is scheduled from 1 February 2027 to 31 January 2030, with a possible 24-month extension extending the term to 31 January 2032. Award decisions will be based on a weighted matrix consisting of price (30%), quality and innovation (20%), improving access and reducing health inequalities (20%), integration and service sustainability (20%), and social value (10%). Participation is open to Small and Medium-sized Enterprises (SME) and Voluntary, Community and Social Enterprises (VCSE).
