Q -- Occupational Health Services - CDC

The Centers for Disease Control and Prevention (CDC), Office of the Director, Office of Health and Safety (OHS), headquartered in Atlanta, Georgia, contemplates awarding a contract to obtain the services of a Contractor to provide a comprehensive program of occupational health services. These services will include (1) In-service physical examinations, (2) Preventative and health promotion programs, (3) Occupational health surveillance programs, (4) Employee counseling services, and (5) Work-site health promotion activities for employees in the Atlanta area; to include the Agency for Toxic Substances and Disease Registry (ATSDR), a sister agency that shares the CDC infrastructure. At CDC's discretion, the Contractor may be a central focus for agency-wide occupational health services which may extend beyond the Atlanta area. Award of a Cost-Plus-Fixed-Fee contract is contemplated, in accordance with FAR Part 15, with Full and Open Competition. The anticipated Period of Performance is as follows: Basic Year - April 1, 2004 to March 31, 2005 with four option years.
