The New York State Office of Mental Health (OMH) announces the availability of funds for the development of one (1) Older Adult & Medically Fragile Support Team to operate in New York City. This team will work in conjunction with the NYC Safe Options Support (SOS) Critical Time Intervention (CTI) Teams to provide specialized services and supports to older adults and individuals with existing chronic medical conditions who are currently unsheltered or recently transitioned from street homelessness to housing. The Older Adult & Medically Fragile Support Team is a multidisciplinary team comprised of an Occupational Therapist, Geriatric Registered Nurse, and Geriatric Social Workers. This team will work in close collaboration with the assigned SOS CTI Team, primary care physicians, specialty care providers, hearing and vision providers, housing providers and others to ensure that SOS members are receiving all services and supports needed to live in the least restrictive environment possible.
