Additionally, bidders must submit evidence of fiscal integrity, such as financial statements from the last two years, and complete several required certifications, including a W-9 and a Conflict-of-Interest Affidavit. Strict packaging and shipping standards are required, including a maximum carton weight of 40 lbs and mandatory palletization for orders exceeding 10 cartons or 200 lbs using Grade A GMA four-way pallets. This is a combined synopsis/solicitation for polypropylene vials with Huntsman 13T25A resin. The Alaska Fisheries Science Center (AFSC), Fisheries Monitoring and Analysis (FMA) division uses these vials for fisheries biological specimen collection at sea off the coast of Alaska. The vials will contain specimens that are hydrated with a glycerol/thymol solution for analysis and for long-term storage preservation. Typical polypropylene vials do not preserve specimens properly due to extreme cold conditions and fluctuating temperatures over time. Vials shall have a Huntsman 13T25A resin so the vials don't crack under cold conditions and maintain long-term storage of specimens. The period of performance is from September 15, 2014 through September 14, 2015 with four (4) one (1) year option periods. All interested concerns must submit a quotation and documentation by September 9, 2014 at 10:00AM Pacific time. All responses must be in writing. A Request for Quote (RFQ) with the technical specifications and clauses is incorporated as an attachment as part of this notice. This announcement constitutes the only solicitation. A firm-fixed-price order may be issued on or before September 15, 2014. **** All contractors doing business with Government shall be registered with the System for Award Management (SAM). Award will not be made to any contractor that is not registered in SAM. The System for Award Management portal is the single source for vendor data for the federal government. For additional information and to register in SAM, please access the following website: https://www.sam.gov/portal/public/SAM/. In order to register with SAM and to be eligible to receive an award, all offerors must have a Dun and Bradstreet number, which may be acquired free of charge by contacting Dun and Bradstreet on line at http://fedgov.dnb.com/webform or by phone at +1 (555) 000-0000. The government encourages all contractors to submit their Representations and Certifications on-line at https://www.sam.gov/portal/public/SAM/.
