Single award term contract for Department of Health & Human Services, Centers for Disease Control, Atlanta, GA. Beginning October 1, 2010, and ending September 30, 2011, with four possible option years. Quality Level 2 is required. These specifications cover the production of of self- and separate-cover pamphlets requiring such operations as electronic prepress, printing (including four-color process), binding, addressing/affixing labels, mailing, and delivery. FREQUENCY OF ORDERS: Approximately 70 - 80 orders per year. QUANTITY: Approximately 6,000 to 30,000 copies per order, with an overall average of 7,170 copies per order. NUMBER OF PAGES: 12 to 300 pages. TRIM SIZE: 8.5 x 11". Deliver f.o.b. contractor's city for mailed shipments and f.o.b. destination for all other shipments, primarily to Altoona, PA, and Atlanta, GA, metropolitan area. Complete production and delivery is required within two workdays for the weekly publication; the balance of the orders typically require delivery within 4 to 10 workdays.
