(I) This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Combined Synopsis/Solicitation Number GS-08P-12-JB-P-0091 is a Sole-Source procurement to Tandus Flooring, Inc. of Dalton, GA in accordance with FAR Part 13. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. (IV) The associated NAICS code is 314110, Carpet and Rug Mills. The small business size standard is 500 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial item(s)/services: CLIN 0001 1 Lot 1000 square yards of C&A "Aragon" Carpet tiles. CLIN 0002 8 Each Four gallon pails of C-36 Primer CLIN 0003 2 Each Four gallon pails of C-14 Releasable Adhesive (VI) Description of requirements is as follows: The Contract Line Item Numbers (CLIN) will be used to replenish existing Owner's stock of replacement carpet tiles for use at the David Skaggs Research Center in Boulder, Colorado. Only Tandus carpet Style #05164, Color 16012, 'Black Opal'; 18" by 18" self adhesive modular carpet tile will match the existing stock, no substitutions will be considered. (VII) Date(s) and place(s) of delivery and acceptance of order is October 26, 2012, General Services Administration, David Skaggs Research Center, 325 South Broadway, Boulder, CO 80305. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items, applies to this acquisition. (IX) FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. Offers from responsible sources may submit a capability statement and quotation which shall be considered by the agency. In the event more than one quote is received, the following factors shall be used to evaluate offers: technical capability of the items offered to meet the Government requirement, price, and delivery. (X) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Oct 2010) with its quote. The offeror shall complete the annual representations and certifications electronically at http://orca.bpn.gov. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition.
