NORTHWOODS NATIONAL CEMETERY Office Trailer Lease Performance of Work Statement General Scope Northwoods National Cemetery has a requirement for a temporary office trailer for a period of twelve months to be provided in accordance with the terms and conditions outlined in this Performance Work Statement (PWS).
Goal The primary objective of this contract is to provide temporary office space for one employee at Northwoods National Cemetery, located at 4520 Lakewood Rd, Harshaw, WI 54529. The contractor will be responsible for providing all required aspects of a 12 month trailer lease, including delivery, setup, and removal.
General Requirements The Contractor shall provide a temporary office trailer for a period of 12 months as well as all transportation, labor, parts, and materials to deliver, setup, secure, dismantle and remove at the end of the lease.
SPECIFICATIONS
The following list, with subsequent corresponding descriptions of services, covers the types and delivery frequencies of services that will be required for fulfillment of this contract. The Contractor shall coordinate with the CO or COR as appropriate to determine delivery schedules, and the trailer placement location.

A.1.1 Delivery and setup of office trailer, skirting and steps: Prior to shipping, the contractor will notify the COR/CO and provide the government with the opportunity to verify the trailer meets the requirements. This inspection can take place either remotely or in-person. The trailer must not be shipped prior to this verification by the government. If the government finds the trailer does not meet the requirements the contractor shall provide an alternative trailer for inspection within 2 weeks (10 business days). The contractor shall provide all resources required to deliver, set up, and properly secure the trailer, skirting and steps. Final inspection and acceptance by the COR/CO will take place when the trailer is ready for occupancy.
A.1.2 Monthly fee for lease: The monthly lease fee must include all reoccurring charges associated with the lease of the trailer including stairs, skirting, and all materials or equipment provided. A.1.3 Dismantling, removal and return: The contractor shall provide all resources required to dismantle and remove the trailer and all associated materials and equipment provided by the contractor. Office Trailer requirements: The contractor shall provide an office trailer of not less than 160 gross square feet that is designed and built for. The trailer must be in new or in as new condition but in no cases more than 3 years old. Trailers that are dirty, stained, discolored, have visible prior damage, or in disrepair will not be accepted. Installation of the office must meet all local and state codes for the town of Harshaw, Oneida County, Wisconsin. The minimum requirements are: 160 gross square feet Three-foot wide exterior door, including hardware. ADA ramp meeting all local, state and Federal ADA regulations for the town of Harshaw, Oneida County, Wisconsin. 2 operable windows that can be opened for ventilation. OSHA approved platform and stairs leading to grade. Skirting the entire perimeter of the office from the floor to the ground that matches the color and material of the exterior of the trailer Exterior finishes shall be visually pleasing to the eye and not detract from the solemnity and decorum of a national cemetery (i.e. Bright colors and large or attention-grabbing designs should be avoided) Provide floor, wall, and roof with insulation as follows: R-19 within the floor, R-13 within exterior walls, R-38 within the roof Interior finishes shall consist of resilient flooring, plywood paneling or painted wallboard on walls, and acoustical tile ceilings. Provide sufficient fluorescent lighting to deliver 70 foot-candles of light at desktop height without the aid of daylight. Provide one light switch Provide one duplex receptacle on each wall. If a wall is 10 feet long or more, provide two receptacles for each 10 feet, or portion thereof, of wall Thermostatically controlled, centralized heating and air conditioning system designed to maintain the temperature between 70- and 80-degrees F with 50 percent relative humidity maintained during the air conditioning season. Provide maintenance or repair services within 72 hours of notification.
Contractor s Representative:
Upon contract award, the Contractor shall promptly provide the Contracting Officer (CO) with the name and contact information of the individual designated as the Contractor s Representative for this contract. The Contractor s Representative shall have overall management responsibility for the execution of all contract requirements and shall serve as the primary point of contact for day-to-day operations, receiving technical direction, and addressing issues that arise under the contract. This individual shall also be responsible for coordinating and communicating matters of mutual concern with Government representatives. The Contractor s Representative may appoint a designated inspector or liaison to work directly with the Government on operational matters subject to the approval of the CO.
Regulatory Compliance, Permits, Licenses:
The Contractor shall obtain and maintain all applicable licenses, permits, and certifications necessary to perform the work required under this contract prior to the commencement of performance. All such licenses and permits shall be current and valid throughout the contract term. Upon request, the Contractor shall provide the Contracting Officer with copies of all licenses and permits. The Contractor shall, at no expense to the Government, be responsible for obtaining and maintaining all necessary licenses and permits and for complying with all applicable Federal, State, and municipal laws, codes, and regulations governing the performance of the contract.
Verification of Service:
The Contractor shall submit all monthly invoices via the Tungsten e-Invoice Network. Copies of each invoice shall be forwarded to the Contracting Officer s Representative (COR) for verification. The COR shall review each invoice to verify that services invoiced conform to contract requirements. In the event services were not delivered or invoiced incorrectly, the COR will document the discrepancies in writing and forward such documentation to the Contracting Officer (CO). The Cemetery certifying official will then contact the Contractor to resolve the issue and/or return the invoice for correction. If the COR does not contact the Contractor regarding any discrepancies within a reasonable timeframe, it shall be assumed the invoice is accurate and approved for payment. The Cemetery will retain copies of all approved invoices for their records and for future reference. Payments shall be made via electronic funds transfer (EFT) through the Tungsten e-Invoice Network following invoice approval.
Inspection of Services:
Definitions: For purposes of this clause, Services includes all services performed, workmanship, and materials furnished or utilized in the performance of the contract. The Contractor shall establish and maintain an inspection system acceptable to the Government covering all services performed under this contract. The Contractor shall maintain complete records of all inspection activities and shall make these records available to the Government during contract performance and for such period thereafter as may be required. The Government reserves the right to inspect and test all services called for by the contract at all reasonable times and locations during the contract term. Such inspections and tests shall be conducted in a manner that does not unreasonably delay the Contractor s performance.
