The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified:
(1) Service-Disabled Veteran Owned Small Business (SDVOSB) (2) Veteran Owned Small Business (VOSB) (3) Small or emerging small business firms (4) Other Than Small business firms
Brand name Agiliti or Equal patient bed rental for the rental of inpatient beds, mattresses, and accessories that will require the contractor to supply rental items for the six (6) VA Medical Centers and locations outlined herein within WI, IL and MI including at inpatient requirements of the facility and the requirements of the Spinal Cord Injury Units and Community Living Centers located at the facilities. The contractor shall provide delivery, set-up, maintenance/service/repair, and pick-up of all items.

Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice; no other copies of this notice will be provided.
Scope of Work: The contractor shall provide rental equipment to the VA Medical Centers and locations outlined herein within WI, IL and MI for the sole purpose of patient care for residents within defined time frames for services. Agiliti Brand Name or Equal

Agiliti: Immerse: Immersion low air loss replacement mattress; width of 35 , 39 , 42 , or 38 ; length of 80 , 82 , or 86 ; low deck height of 10 ; weight capacity of 550 lbs or 950 lbs. Alternate/Mighty Air: Alternation low air low loss replacement mattress with alternation and immersion mode; width of 35 , 39 or 38 ; length 80 or 82 ; low deck height 8 or 9 ; weight capacity 550 lbs. or 950 lbs. Agiliti Rotate/Big Turn Mattress: Low air loss mattress replacement with pulsation and immersion capabilities as well as percussion/vibration; Width 35 , 39 , or 48 and length 80 or 82 ; patient weight 550 lbs. or 950 lbs. weight capacity. Include a Power-drive System. IV poles that will support IV pumps and bags.
Bariatric Size Bed/Unit: Each Bed/Unit must: Be electric. Include a built-in scale. Have an adjustable headrest. Be able to properly accommodate a low air loss surface. Be capable of supporting a patient with a weight up to 1,000 pounds. Have a pressure redistribution-type surface/system to help turn the patient. Be able to protect and decrease pressure, shear, friction, and control moisture. Have a retractable foot mechanism to assist with length adjustment. Have at a minimum, safety features that alarms to power failures. A main power shut off switch in the event of an emergency. Height requirement- at least six inches from the floor with the bed/unit at its lowest level. Width requirement- 37- 48 inches. Length requirement- up to 86 inches. Weight requirement minimum 1000. IV poles that will support IV pumps and bags.
Bariatric Size Bed/Unit for the Intensive Care Unit (ICU): Must include: In addition to all requirements of Bariatric Size Bed/Unit, must: Have a percussion and vibration functionality. Have continuous lateral rotation therapy functionality. Height requirement- at least six inches from the floor with the bed/unit at its lowest level. Width requirement- 37 to 48 inches. Length requirement- up to 86 inches. IV poles that will support IV pumps and bags.
Critical Care Bed for the Intensive Care Unit (ICU): Each bed/unit must: Accommodate patient s weight up to 500 pounds. Have percussion and vibration functionality. Have continuous lateral rotation therapy functionality. Accommodate a low air loss type surface/mattress. Have footboard extenders. Have side extender. IV poles that will support IV pumps and bags. Height requirement- at least six inches from the floor with the bed/unit at its lowest level. Width requirement up to 36 inches. Length requirement- up to 86 inches.
Air fluidized Therapy Unit: The beds/units must: Prevent/treat Stage I, II, and complicated Stage III/IV pressure ulcers. Accommodate patients up to 500lbs. Have a height adjusting mechanism to allow the unit to be lowered and raised.
Air Fluidized Therapy Unit for the Spinal Cord Injury Unit (SCIU): The bed/units must: Be air fluidized therapy units with headed beads.
Low Air Loss Surface: The surface/mattresses should be of a foam type or air type, static mattress. Foam base must be at least 2.5 inches. Surface must be able to be placed directly on a hospital bed frame. The surface/mattress must be able to prevent/treat Stage I, II, and uncomplicated Stage III/IV pressure ulcers. The surface/mattress must be capable of providing alternating pressure. The surface/mattress must be able to accommodate medical centers existing bed frames. Width requirement up to 36 inches. Length requirement up to 86 inches.
Fluid Immersion Simulation Therapy for the SCIU: The surface/mattress must: Accommodate bariatric patients (up to 1000lbs) and standard patients (up to 500lbs). Be able to accommodate existing bed frames in the medical centers. Support bariatric patients with advance stage wounds, multiple pressure ulcers, flaps, skin grafts, and burns. Width requirement 36 - 48 inches. Length requirement up to 86 inches. Floor Mat: Structural features include a bonded top cover to help transfer impact forces outward, A high-density composite core to absorb impact. A non-slip base to secure the mat. A tapered ground perimeter to help maintain staff and patient safety.
