WSF is purchasing occupational medical surveillance services to support WSF’s occupational health and safety programs. Services will include regular examinations, testing, evaluation and record keeping that aligns with WA State regulatory framework and HIPPA compliance. Services will be provided to employees working on vessels, in terminals and maintenance and thus requires flexibility and predictability when scheduling. All medical examinations and evaluations must be performed by or under the supervision of a licensed physician who is board certified in occupational medicine. The contracted services must submit reports that provide verification of exam, written opinion or clearance letter, confirmation of employee medical condition that would increase impairment during work activities, statement confirming physician has informed employee of medical exam results and conditions, recommendation for changing medical exam frequency, signification findings, and provide submission of this document to WSF Health and Safety and a copy to employee within 60 days of exam.
