STATEMENT OF WORK Occupational Safety and Health Administration (OSHA) Salt Lake Technical Center (SLTC) 1. INTRODUCTION The Salt Lake Technical Center (SLTC) analyzes samples submitted by Federal Compliance Safety and Health Officers (CSHOs) from workplaces all over the United States and the following territories: Puerto Rico, Virgin Islands, and Guam. Analytical results provided by SLTC to CSHOs are often used as the basis for citations and these results must be accurate, precise, and defendable in court proceedings. 2. SCOPE OF WORK The vendor will provide maintenance of the Rigaku D/Max 2500 X-Ray Diffraction Instrument, located and the Salt Lake Technical Center Laboratory. Maintenance will include parts and labor for repairs needed. With One (1) onsite service call within the 12 month contract period, this will include all labor, travel and living expenses. The Vendor will provide all parts required for repairs. Vendor will also provide Technical phone support Monday - Friday during normal business day operations, excluding the contractor recognized holidays and government recognized holidays see list below. Vendor will respond to maintenances requests within 72 hours. 3. STATEMENT OF WORK All repairs of X-Ray Diffraction (XRD) equipment listed in #2 "Scope of Work" will be ordered through a Delivery/Repair Order. The contractor will have the knowledge and experience needed and have access to the parts required to repair and service Rigaku XRD equipment as needed to maintain operation at the OSHA Salt Lake Technical Center, Sandy, Utah. The serial number YD232205A, DY232205, YD09601, and YD09602. 4. DELIVERABLES The vendor will provide the needed parts for repairs and a Rigaku trained technician to perform all work needed to maintain this instrument. The vendor will respond to requested technical assistance with 72 hours of the request.
