INTENT TO SOLE SOURCE FAR 13.106(B) This is a notice of intent of The Louis Stokes Cleveland Department of Veterans Affairs Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106 to negotiate sole source with Terumo Cardiovascular Group for supplying The Department of Veterans Affairs Medical Center, Cleveland, Ohio, Medical Service, with: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QTY UNIT 86022 CSZ Hemotherm 3 EA These units are for use in operating rooms at Louis Stokes Cleveland VA Medical Center. The Cleveland VA Medical Center, surgical services has a need to replace our existing 3 heart-lung heating/cooling systems. The FDA and CDC have determined our currently owned heater/cooler systems to be a health threat to the cardiac surgery patient and operating room staff. (1) STATEMENT OF CAPABILITY: Submit a brief description if you believe your company is capable of fulfilling this requirement, five (5) pages or less, that demonstrates how your company has the capabilities with sufficient detail to allow for a comparison with the recommended product and experience to provide the Surgical Supplies described above. If a FSS/GSA contract schedule holder, please provide contract schedule number. Please specify your availability to delivery schedule and your address. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please (i) NOTE that any future solicitation could only be awarded to a contractor who is registered in SAM and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. The Government believes that only one business can satisfy the agency requirements and intends to negotiate with only one source under the authority of FAR 13.106(b) - access to proprietary information. Other persons may identify their interest and capability to respond to this requirement by Friday January 13, 2014 at 3PM EST. Such capabilities information will be used solely for the purpose of determining whether or not to conduct this procurement on a competitive basis. All information shall be furnished at no cost or obligation to the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
