This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, C.W. Bill Young VAHCS has a need to procure (Brand Name Only), OLYMPUS AMERICA, INC.; In-Accordance with FAR 13.106-1(b), solicit from a single source for purchases not exceeding the simplified acquisition threshold: Fourteen (14) each MAJ-1545: MAJ-1545 RFID SCOPE TAG AND 2 BANDS and Fourteen (14) each LF-GP: LF-GP TRACHEAL INTUBATION FIBE RSCCOPE-PO for the Medicine Service. The RFQ number is VA248-17-Q-0589. This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2005-95 (01/19/17). The combined synopsis solicitation is 100% Set-Aside for Service Disabled Veteran Owned Business (SDVOSB) and Veteran Owned Small Business (VOSB). Preference may be given to SDVOSB.
