This contract is a Small Business set-aside solicitation issued as a combined synopsis/solicitation for the procurement of two key items: the Next Generation 2-Dimensional MapCheck system and the MapPHAN 2 water equivalent phantom. These items are specifically brand name Sun Nuclear products, which are critical for the Radiation Therapy Service at the Boston VA Medical Center to ensure accurate quality assurance (QA) in patient treatments. The MapCheck system is a real-time diode array that interfaces with treatment planning software to compare calculated and measured dose files, essential for verifying that radiation doses administered to patients are within prescribed tolerances. The MapPHAN-2 phantom supports the MapCheck device at specific measurement depths, enabling precise QA in different orientations. The procurement follows federal acquisition regulations and is conducted under FAR Subpart 12.6 as a Request for Quotation (RFQ), with responses due electronically by May 29, 2015. It is governed by relevant FAR and VA Acquisition Regulation clauses and includes provisions for accelerated payments to small business subcontractors and warranty and inspection requirements. The contract is justified as a sole-source acquisition despite competition requirements, due to the need for continuity with the existing Sun Nuclear system in use, maintaining the established QA program to avoid treatment delays and retraining. Delivery is FOB destination to the Boston VA Medical Center, with pricing inclusive of all delivery costs. Evaluation factors focus on price and conformity to the brand's specifications to ensure the government receives the most advantageous offer.

The total value of this order shall not exceed $49,149.00 JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). PURCHASE REQUEST OR REQUISITION NUMBER: 523-15-3-7821-0167 1A. PROJECT/TASK NUMBER 1B. ESTIMATED AMOUNT: $49,403.00 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: MapCHeck 2 Next Generation (1527 channel) 2-Dimensional real time diode array. MapPHAN™-2 (5cm) Water equivalent phantom securely holds MapCHECK® 2 array at measurement plane depth of 5cm and backscatter depth of 5cm. Measurement may occur in the coronal and sagittal positions 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: The Radiation Therapy Department currently utilizes the MapCheck system for the Quality Assurance of each patients treatment. It is necessary to assure that each plan created for each individual patient is accurate and with in 2% of the Dr.'s prescribed dose. With this updated version we will be able to continue to QA many of the new advancements in treatment being performed here at the VA. Also, the MapPHAN is necessary to QA the MapCheck device making sure it is within the necessary specs to accurately QA patients data. 4. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: The Radiation Therapy Service has been utilizing the MapCHeck system for many years. There is a solid program set up used to maintain safe and accurate treatments to our veterans. Changing to a different QA system would cause a delay in treatments due to the need to start over a QA program with new devices and establish new data, knowledge and training. 5. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: The Physics department with in the Radiation Therapy Service has been utilizing this QA system for many years. They obtained the quote for the system. This QA system is necessary to continue the QA program set up here at the Boston VA Medical Center and maintain a high quality of treatment for each veteran. 6. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
