This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W81K00-06-T-0454 is being issued as a Request for Quotation. The RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. The procurement is full and open, NAICS code is 339113, small business standard size is 500 employees. Requirement is for 2 each Olympus Video Colonoscopes #CF-H180AL or equivalent; 4 each Olympus Pediatric Video Colonoscopes, #PCF-140L or equival ent; 2 each Olympus Video Gastroscopes #GIF-H180 or equivalent; and 2 each Olympus Exera II Video Bronchoscopes #BF-Q180 or equivalent. Includes Shipping, 2 each Operators Manual, 2 each Maintenance Manuals, Standard Commercial Warranty on parts and labo r for each item. Include product specifications for each product. Items to be shipped to Evans Army Community Hospital, Fort Carson, CO 80913 within forty-five days after receipt of order.
