65--Sole Source Synopsis - Immobilization System

Veterans Affairs Network Contracting Office 6 (NCO 6) intends to negotiate on a sole source basis with CIVCO Radiotherapy (DUNS: 134614411), pursuant to FAR part 12, FAR part 13, and specifically FAR 13.106-1(b)(1), for purchases not exceeding the simplified acquisition threshold. The proposed sole source purchase order is for patient immobilization equipment for radiology, manufactured by CIVCO Radiotherapy. The anticipated CLIN structure, quantities and manufacturer part numbers are provided in the attached document. The requiring Medical Center for this action is the Hunter Holmes VA Medical Center in Richmond, Virginia. The NAICS code for this contract action is 339113, Surgical Appliance and Supplies Manufacturing (SBA Size Standard: 750 employees). The Product Service Code for this action is 6515, Medical and Surgical Instruments, Equipment, and Supplies. The facility requires components that comprise a patient immobilization system for use in the radiology service. The facility staff evaluated three brands of equipment, including IZMED Medical (on 9/7/2016), QFIX Medical (on 9/21/2016) and CIVCO Medical Solutions (on 1/17/2017). Based on the product demonstrations, facility staff identified the product from CIVCO Radiology (CIVCO Medical Solutions) as best fitting facility needs. Further market research was conducted to identified small business distributors of CIVCO Radiology products. The only identified source resulting from a FBO.gov sources sought notice, FPDS-NG search, and VIP Search was the manufacturer, CIVCO Radiology (DUNS: 134614411). This notice of intent is not a request for competitive proposals; however, any responsible source who believes it is capable of meeting the requirement, may submit a capability statement to the contracting office no later than Wednesday, June 7, 2017, 08:00PM EST.
