DEPARTMENT OF VETERANS AFFAIRS NETWORK 22 LOGISTICS OFFICE JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION FAR 6.302-1, 41 U.S.C. 253 (C) (1) CONTRACTOR: CARL ZEISS; PR 691-10-3-685-0090 1. Agency and Contracting Activity Identification: The VA Network 22 Logistics Office, 5901 E. 7th Street, Long Beach, California, as directed by the VA Greater Los Angeles HealthCare System (GLAHS), will enter into a Purchase Order that requires justification for other than full and open competition. 2. Nature of Acquisition Being Approved: Specialized equipment emergent orders requested for the VA GLAHS and for the community clinic at Bakersfield, CA. This equipment is unique insights to support clinical detection, diagnosis and ongoing management that satisfy the VA's stringent requirements while still being deliverable within mandatory allotted time frame. 3. Description of Requirement: The awarded contractor is to provide a customized Humphrey Field Analyzer. Additional equipment for the VA GLAHS Ambulatory Service and CBOC Bakersfield. The estimated cost is $42,688.00. 4. Statutory Authority Permitting Other Than Full and Open Competition: The equipment is being procured under the authority of 41 U.S.C. (c ) (2), FAR Part 6.302-1 (a). 5. The Nature of the Acquisition Requires Use of the Authority Cited: The Humphrey Field Analyzer is an essential part of many types of eye examinations, especially for glaucoma, traumatic brain injury, stroke and others of neurological visual dysfunction. This item is unique to the manufacturer due to the standard that expands the potential of perimeter with new technologies. Carl Zeiss allows this product to meet the very stringent standards of VA healthcare policy. All VA GLAHS clinics only used this type of field analyzer. This is the only field analyzer that has been connected to GLA Vista Imaging system for transfer and storage of test results. Carl Zeiss Meditec is the only company that makes and supplies this model and the only analyzer that is compatible to the system that is being used at the VA hospital and the community clinic. The VA WLA is requesting sole source procurement of the Humphrey Field Analyzer for compatibility to the system they are now using. Only one responsible source and no other supplies or services will satisfy agency requirements. 6. A Description of Efforts Made to Ensure That Offers From as Many Potential Sources as Practicable and Whether a Notice Will Be Publicized. See number 5 above. A notice has been publicized for this procurement. 7. Anticipated Cost: Estimated cost considered fair and reasonable compared to products that are not customized to meet the VA's needs though are somewhat similar to the product being purchased. 8. Result of Market Survey: Market research indicates that the product is unique due to the customization added to the product to ensure compliance with VA standards and other medical standards. This product could not be found within GSA schedules though an- upgraded versions could be found with other companies, none are suitable for the task at hand. 9. Any Other Facts Supporting the Use of Other Than Full and Open Competition: None. See item 5 above. 10. Any Other Interested Sources? There are no other sources that have expresses an interest to date. 11. A Statement of the Actions, if any, the Agency May Take To Remove of Overcome Any Barriers to Competition Before Any Subsequent Acquisition is required: It is not anticipated that any subsequent requirement will be needed by the Government. 12.
