The Department of Veterans Affairs (VA) is seeking sources capable of providing Frozen Intravenous (IV) Antibiotics to VA Loma Linda Healthcare System Pharmacy Service. STATEMENT OF WORK Background: The Department of Veterans Affairs Loma Linda Healthcare System Pharmacy Service intends to set up a firm fixed price Indefinite Delivery Indefinite Quantity (IDIQ) contract of various frozen intravenous (IV) antibiotics listed below for direct patient care patient care for base plus option four years. These pharmaceuticals are necessary for the treatment of acute or life-threatening conditions.

Requirements: Medication must be FDA-approved and conform to VA National Formulary standards. Product must have a minimum remaining shelf life of [e.g., 6 months] at time of delivery. Delivery must occur within [e.g., 1 3 business days] of order placement. The vendor must comply with VA procurement policy, including Drug Supply Chain Security Act (DSCSA) standards. Products must include a certificate of analysis or equivalency upon request.
Delivery Requirements: Must be delivered to designated units as ordered for medication. Must provide shipment tracking and documentation. Must submit invoices on Tungsten with matching the delivery order Must provide Proof of shipment and delivery confirmation upon request.
