NOTICE OF INTENT TO SOLE SOURCE Supplies: Glucose Accu-Check Inform II test strips THIS IS NOT A REQUEST FOR COMPETITIVE QUOTATIONS OR PROPOSALS The Department of Veteran Affairs (VA), VISN 17 Network Contracting Office intends to award a sole source contract for the Dallas VA HealthCare System Pathology and Laboratory Department. This proposed contract action is to procure Glucose Accu-Check testing Strips for Dallas VA HealthCare System Pathology and Laboratory Department. Base Period: 06/01/2024-05/31/2025 Option Year 1 : 06/01/2025-05/31/2026 Option Year 2: 06/01/2026-05/31/2027 Option Year 3: 06/01/2027-05/31/2028 Option Year 4: 06/01/2028-05/31/2029 Classification Code: 6550 NAICS: 325413 Government intends to award a BASE + 4 fixed-price option contract to Roche Diabetes Care, Inc, 9115 Hague Rd., Indianapolis, Indiana 46250. This acquisition is conducted under the authority of 41 U.S.C. 1901 Simplified acquisition procedures as implemented in FAR 8.405-6(a)(1)(B), only one responsible source and no other supplies or services will satisfy agency requirements.
