Description of Requirements for the items to be acquired; Statement of Work 1. Purpose: 1.1 The Department of Veterans Affairs, Eastern Colorado HealthCare System, Rocky Mountain Regional Hospital is requesting a Firm-Fixed Contract for the delivery and installation of two (2 EA) bariatric beds with mattresses and twenty-five (25 EA) hospital beds with mattresses. Removal and disposition of existing beds and mattresses is also required. Contractor shall also provide onsite training in advance of delivery of requested equipment and supplies.
2. Salient Characteristics: 2.1 The Low Profile Bariatric Beds need to have the following salient characteristics: 2.1.1 Provide patient capacity of at least 800 lbs with integrated scale. 2.1.2 Low bed height to minimize fall risk of 12 inches or less. 2.1.3 Minimum three position side-rails with adjustable side rail width from 36 to 48 . 2.1.4 Brake pedals at head and foot of bed. 2.1.5 Integrated siderail and footboard controls that allows for bed repositioning, bed exit feature, scale, and lock-out function. 2.1.6 Digital bed status indicator on home screen, displays bed exit/alarm, low bed indicator, HOB angle, brake set. 2.1.7 Aux 110V and USB outlet. 2.1.8 Patient bed controls including call button, adjustable bed repositioning. 2.1.9 CPR electronic and manual controls. 2.1.10 Trapeze bars to allow residents to independently shift themselves in bed for comfort and to reposition themselves to prevent pressure ulcers. The trapeze bars need to have the following salient characteristics: 2.1.10.1 Provide patient capacity of at least 500lbs. 2.1.10.2 Attaches to and mounts securely to headboard of the bariatric bed. 2.1.11.2 Overhead bar and handle height are adjustable to allow for optimal reach to allow resident to reposition self in bed.

2.4 The hospital mattresses need to have the following salient characteristics: 2.4.1 Gel support surface that redistributes weight to prevent pressure injuries and skin breakdown. 2.4.2 Microclimate system to promote airflow. 2.4.3 Integrated pump system that allows for body repositioning and pressure ulcer prevention. 2.4.4 Turn assist feature that assists with turning, lifting and offloading patients. 3. 3.1 Contractor shall deliver all equipment and supplies to the following address: Pueblo VA Community Living Center (Extended Care) 2600 Oakshire Lane Pueblo, CO 81008
3.2 Delivery Hours of Operation: Monday Friday; 0800hrs 1500hrs; excluding all Federal Holidays and any other day specifically declared a National Holiday by the President of the United States.

3.3 Disposal of Existing Equipment: Up to six (6) beds per day shall be delivered over a five (5) day period. As each bed with mattress is delivered, a bed and mattress shall be removed for disposition.
3.4 When one of the above designated legal holidays falls on a Sunday, the following Monday will be observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday by US Government Agencies.
3.5 There is no loading dock available. Delivery Truck must have an operable lift gate. Patient room doorway dimensions are approximately 44.75 W x 82.75 H. Hallway dimensions are See attached floor plan for additional details.
3.6 No onsite storage is available during installation period. Contractor shall be responsible for storage of beds during installation time period.
3.7 The Contractor shall verify and inspect all items. Any and all deficiencies (damage/overage/shortage) shall be brought to the attention of the delivery POC in writing, preferably via email, immediately upon identification. Contractor shall repair or coordinate with the COR for replacement of damaged, defective, or missing items.
3.8 Contractor shall uncrate all items received and perform all required assembly in accordance with the manufacturers instructions. Contractor is responsible for identifying additional hardware and tools required to complete assembly and installation.
3.9 Contractor will be responsible for the removal and disposal of all trash/debris connected with uncrating and assembling all items installed under this contract.

5. Training: 5.1 End user training sessions must be scheduled following award and in advance of delivery of beds. Dates and times to be scheduled with POC upon award. Training must occur prior to delivery of order to ensure staff are trained and ready to use equipment upon delivery. Training shall be onsite at delivery location.

52.212-1, Instructions to Offerors--Commercial Items applies to this acquisition ADDENDUM to RFO 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufacturers or gray market items will be acceptable.
Offeror shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and services associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.
The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet or exceed are specified in the solicitation. To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must Meet the salient physical, functional, or performance characteristics specified in this solicitation; Clearly identify the item by- Brand name, if any and Make or model number, Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. The contracting officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the contracting officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. Offerors are warned against contacting any VA personnel other than the Contracting Officer and Contract Specialist prior to placement of any award made resulting from this RFQ. All proprietary information shall be clearly and properly marked. If the Offeror submits annexes, documentation, attachments or the like, not specifically required by this solicitation, such will count against the Offeror s page limitations unless otherwise indicated in the specific volume instructions below.
All information shall be confined to the appropriate file. The offeror shall confine submissions to essential matters, sufficient to define the quote and provide an adequate basis for evaluation. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each quote.
Submission of quote shall include the following volumes: (I) Technical capability or quality of the item offered to meet the Government requirement; (II) Price; (III) Completion of RFO 25 Provision RFO 52.225-2.
Volume I - Technical capability or quality of the item offered to meet the Government requirement The offeror shall submit specifications, cut sheets, or brochures confirming the equal products, including equal products of the brand name manufacturer submitted, meets or exceeds the salient physical, functional, or performance characteristic specified in this solicitation. The product(s) quoted must be identified by brand name, if any, and make or model number.
Volume II Price Price - The offeror shall complete the pricing schedule provided in Section (v) above: Price/Cost Schedule.

ADDENDUM to RFO 52.212-2 EVALUATION COMMERCIAL ITEMS:
The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government considering price and other factors.
The following factors shall be used to evaluate quotations: Factor 1. Technical capability or quality of the item offered to meet the Government requirement Factor 2. Price Factor 3. Completion of RFO 25 Provision. RFO 52.225-2. Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process, where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation, but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following:
Factor 1. Technical capability or quality of the item offered to meet the Government requirement: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation.
Factor 2. Price: The Government will evaluate the price by adding the total of all line item prices. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offers that do not meet or exceed the technical capability or quality of the item offered to the Government requirement shall not be selected regardless of price.
Factor 3. Completion of RFO 25 Provision: The Government will evaluate the responses an offeror makes to Attachment - RFO 52.225-2 for completeness. The Government will evaluate quotes of foreign and domestic offers IAW RFO Part 25. The offeror will complete the provision if required IAW RFO 25. RFO 52.225-2.
