ACCESSIBILITY HOME MODIFICATION – NEW SMYRNA

The Florida Department of Education, through its Division of Vocational Rehabilitation, has awarded a contract under solicitation ITB 2026-037 for accessibility home modifications at a modular residential structure to support a DVR participant. The scope of work includes constructing a new wooden deck at the back entrance, replacing the existing tub/shower in the bathroom, and installing one threshold ramp in the home office. All work must comply with the Americans with Disabilities Act Accessibility Guidelines, applicable state and local building codes, and the detailed specifications outlined in Attachment G. The contract is for a six-month term and requires the provision of all labor, supervision, materials, hardware, permits, inspections, and drawings necessary to complete the modifications. Pricing must include all costs for labor, transportation, incidental services, and materials, with no additional charges permitted, and bids must remain firm for 180 days from the bid opening date. Delivery and performance are subject to FOB Destination terms, and all work must be completed within the contract period, with time being of the essence — any delay constitutes a material breach. The contractor is required to maintain comprehensive general liability insurance with a minimum of $1,000,000 coverage and must list the Department as an additional insured, along with workers’ compensation insurance meeting the minimum standards under Chapter 440 of the Florida Statutes: $100,000 per accident, $100,000 per person, and a $500,000 policy aggregate. Insurance policies must be issued by carriers with a Best’s rating of “A” or better and include a 30-day cancellation notice provision. The contractor must indemnify the Department against all third-party claims and maintain a drug-free workplace as required under Attachment D. All bidders were required to submit a completed Vendor Certification Form (PUR 7801), Disclosure Statement, and, where applicable, the Minority Subcontractor Utilization Summary and Foreign Country of Concern Attestation. Performance is subject to inspection and acceptance by the Department, with deficiencies addressed through written notification and correction requirements; accepted deliverables must meet specifications or be replaced at the contractor’s expense. Invoicing must occur within 30 days of delivery and acceptance, accompanied by the Utilization Summary form, and payments are processed by the Department of Education Comptroller in accordance with Florida Statute 215.422.
