This acquisition is a 100% Indian Small Business Economic Enterprise (ISBEE) set-aside under the Buy Indian Act. This solicitation incorporates provisions and clauses by reference.  The full text of provisions and clauses may be accessed electronically at www.acquisition.gov and www.acquisition.gov/hhsar.


Offerors shall review the attached Combined Synopsis/Solicitation for complete requirements, submission insturctions, evaluation factors, and terms and condititions. Attachment 1 - Laboratory Supply Evaluation Schedule provides the representative items for quotation and evaluation, and Attachment 2 - Stattement of Work (SOW) provides the Government's requirements. The supply list is representative and not all-inclusive; order will be placed on an as-needed basis based on actual laboratory requirements.


Brand Name: Unless otherwise specifically identified in the SOW or Attachment 1, manufacturer names and part numbers are provided for identification and evaluation purposes. Equivalent products may be offered, subject to Government determination of technical acceptability.



The period of performance (or delivery) is:


Base Period: October 1, 2026, through September 30, 2027

Delivery shall be FOB Destination to the Crownpoint Healthcare Facility, Crownpoint, New Mexico 87313


Supplies shall be delivered within the delivery times proposed in the Contractor’s quotation and accepted by the Government. The Contractor shall promptly notify the Government of anticipated delays or backorders.
