CCC Tahoe Center: Pre-Enrollment Physicals

The contractor is required to provide on-site pre-enrollment physical examinations for all California Conservation Corps (CCC) Trainees at the Tahoe Center located at 1949 Apache Ave, South Lake Tahoe, CA, to determine fitness for both Fire Crew and Standard Corpsmember positions. Services must be delivered by licensed physicians, nurse practitioners, or physician assistants in compliance with California state medical board regulations and the CCC 248 Medical/Position Analysis Form. Evaluations must document assessments of vital systems including cardiovascular, neurological, musculoskeletal, and mental health, using official CCC forms CCC 126, 126-A, and 126-B, and culminate in one of four medical determinations regarding the trainee’s ability to perform essential duties with or without accommodation. All examinations and records must adhere to confidentiality standards including HIPAA, and documentation must be submitted to the CCC Health and Safety Unit in Sacramento for final acceptance. The contract is funded at a fixed estimated value of $30,000, split evenly between fiscal years 2026/27 and 2027/28, with no authorized line-item pricing beyond this cap. Payment is processed manually through paper invoices submitted to the CCC Accounting Claims Section at the Tahoe Center address. Performance must begin upon contract execution and continue through June 30, 2028, with a possible one-year extension and up to a 30% value increase capped at $250,000 if scope remains unchanged. The contractor must maintain primary insurance coverage with an AM Best rating of at least A- and Class VI financial size, and all personnel must be cleared for state property access with no criminal history involving theft, drug abuse, or abuse of persons. Subcontracting requires prior written state approval, and the prime contractor retains full liability for all subcontractor actions. The state reserves the right to terminate for cause, including conflict of interest, breach of confidentiality, or violation of the weapons prohibition on state grounds.
