CCC Ventura Training Center: Pre-Enrollment Physicals

The California Conservation Corps is seeking a contractor to provide pre-enrollment physical examinations for Corpsmember Trainees at the Placer Center and other locations within California, with performance obligated from July 1, 2026, through June 30, 2029. The contractor is responsible for delivering all labor, materials, tools, and incidentals required to conduct physicals in accordance with the Participant Position Analysis and the Trainee’s completed Health Questionnaire, evaluating all applicants for both CCC Fire Crew and Standard Corpsmember positions regardless of program interest. Deliverables include the submission of the completed Health Questionnaire (CCC 126), the CCC Medical Report (CCC 126-A), and the Pre-Enrollment Exam Summary (CCC 126-B). The contract is governed by California law, subject to the State’s General Terms and Conditions, which are non-negotiable, and includes clauses for indemnification, termination for cause, and a 30-day termination without cause notice period. All records created under this contract are considered public records under the California Public Records Act. The maximum contract value is capped at $45,000, with payment made monthly in arrears upon submission of detailed, approved invoices that include itemized services, time periods, agreement number, and total amount due, sent to the Contract Manager at the CCC Ventura Training Center. The contractor must maintain automobile liability insurance with a $1,000,000 combined single limit, workers’ compensation coverage with $1,000,000 employer’s liability limits, and name the State of California as an additional insured; insurance must be primary, not excess, and issued by carriers with an AM Best rating of at least A- and financial category VI.
