CHIP and CHA implementation, Governance, Community Engagement, and Data Integration Support

The Community Health Assessment (CHA) is a countywide process that will combine quantitative data, qualitative findings, lived experience, community assets, and stakeholder input to identify priority health and quality-of-life needs in San Mateo County. The CHA will provide the evidence base for a subsequent Community Health Improvement Plan (CHIP), which will identify shared priorities, strategies, responsible partners, measures, and accountability mechanisms. The County has completed initial planning and now requires limited, specialized consulting support to move from preliminary planning to implementation readiness. The contractor will advise County staff and facilitate structured planning activities related to governance, community-engagement readiness, CBO partnership structures, and data-integration approaches. The contractor will work collaboratively with County staff, County-designated CHA/CHIP governance bodies, trusted-messenger CBOs, the CHIP backbone organization, and other County-identified stakeholders. The contractor's role is advisory and capacity-building. County staff and County-contracted CBO partners will retain responsibility for operational outreach, fieldwork, participant engagement, data custody, and final CHA/CHIP products.
