This requirement is in support of the 86th Medical Group's (MDG) Behavioral Health Optimization Program (BHOP). The contracted services shall include health consultations and health care facilitation in a specialty mental health clinic or in a patient centered medical home (PCMH) location,consultation services to the primary care managers (PCM), and delivery of behavioral health interventions in the primary care clinic that support PCM and patient treatment goals. The Contractor shall provide a qualified personnel to perform services in accordance with (IAW) this Performance Work Statement (PWS), and the Joint Commission on Accreditation of Healthcare Organization (JACHO). The Contractor shall provide a qualified/fully certified Behavioral Health Consultant and/or Behavioral Health Care Facilitator to perform Clinical Psychologist Support Services at the 86th Medical Group, Ramstein Air Base Please see attached Market Research Questionnaire and revised Performance Work Statement (PWS). Please note that the PWS is a draft that will be revised and standardized during the ongoing market research and finalized prior to release of a solicitation. The Government is seeking responses to this Market Research Questionnaire to determine potential sources and obtain valuable information for the development of the acquisition.
