Proposers must also propose the most suitable dental network for the Town and include a detailed network disruption analysis. To be eligible, vendors must demonstrate adequate financial resources, delivery capability, a satisfactory track record of integrity and ethics spanning at least five to seven years, and current licensing and certification, as well as provide a completed Certification Form and Conflict of Interest Questionnaire.
