The Indiana National Guard (INARNG) United States Property Fiscal Office (USPFO) Purchasing and Contracting (P&C) is issuing this combined synopsis/solicitation for Surgical Dental Services.
THIS WILL BE AN UNPRICED PURCHASE ORDER, IAW FAR 12.201-1(e)(2), WITH A NOT TO EXCEED AMOUNT OF $100,000. THE SOLDIERS WILL BE SENT TO YOUR FACILITY WHEN SERVICES ARE NEEDED AND ONCE COMPLETED YOU WILL BILL THROUGH WAWF.
YOU MUST BE ABLE TO PROVIDE SURGICAL DENTAL SERVICES WITHIN 50 MILES OF CAMP ATTERBURY INDIANA. IF YOU CAN NOT PROVIDE THIS SERVICE IN THIS LOCATION YOUR RESPONSE WILL BE CONSIDERED NON-RESPONSIVE. MUST BE ABLE TO PROVIDE SURGICAL EXTRACTIONS, CROWNS, ROOT CANALS, AND ASSOCIATED PROCEDURES. As stated in the Performance of Work Statement (PWS), the deliverables include: 1) Extensive dental treatment; 2) Monthly tracker of treatment received and costs; and 3) Completed DD 2813's showing the soldiers dental readiness code following treatment. A complete list is included in the Performance Work Statement.
YOU MUST ACCEPT CONCORDIA AND BLUE SHIELD BLUE CROSS INSURANCE. SOLDIERS MAY HAVE SOME INSURANCE AND IT WILL BE BILLED FIRST. THE REMAINING BALANCE OR IF THE SOLDIER HAS NO INSURANCE THAN THE GOVERNMENT WILL INVOICED VIA WAWF.
YOU MUST HAVE EXPERIENCE IN THIS TYPE OF CONTRACT AND PRICING ARRANGEMENT. MUST HAVE EXPERIENCE IN GOVERNMENT CONTRACTING.
WE DO NOT SET UP WEEKEND EVENTS. ONCE A SOLDIER HAS BEEN IDENTIFIED TO RECEIVE TREATMENT A VISIT WILL BE SCHEDULED WITH TREATMENT DETAILED.
If you are interested and would like to submit a proposal provide exact location of your office where services will be provided. Provide past experience with government contracts, and if possible with this type of payment arrangement. Your abililty and acceptence of the named insurances. Also submit retail price sheet or fee sheet. We realize there are wild variations in pricing so we are just asking for the basic price to complete the task with no complications.
The evaulation of the proposals will be on location, past experience, the acceptance of the insurance, and ability to complete all PWS tasks. Pricing will only be used to verify a fair market price. Its weight will be minimal.
