This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-21-Q-0105 NAICS Code: 423430, Size Standard: 250 Set Aside: 100% Buy-Indian Contractor shall provide a firm, fixed-price quote to provide Desktop Computers for the Haskell Indian Health Center. The Period of Performance for this requirement is 60 days from the Award Date. Questions are due no later than August 3, 2021 at 3PM CST. The closing date for receipt of quotes is August 16, 2021 at 4PM CST. Offers shall be submitted to the Oklahoma Area Indian Health Service, Division of Acquisition Management, 701 Market Drive, Oklahoma City, Oklahoma 73114 or emailed to contact@example.gov . Award decision shall be based on lowest technical accepted price. The contract shall be awarded to the Lowest Priced Technically Acceptable (LPTA) offeror. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote B) Technical Specifications (submittals) of your proposal along with Country of Origin C) Verifiable relevant, (2) recent past performances that include similar experience in deploying hardware similar in SOW. Include contracting organization, contract POC (phone number and email) and contract amount. Proposals shall be evaluated on an acceptable/non-acceptable basis. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability and past-performances beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS OF THE SCOPE OF WORK.
