AI Contract Overview

The Ohio Department of Health's Bureau of Health Improvement and Wellness has secured a cooperative agreement from the Centers for Disease Control and Prevention to support the Cardiovascular Health (CVH) Program through June 2028. This initiative aims to address the high burden of cardiovascular disease, stroke, diabetes, and related chronic conditions in Ohio, which ranks poorly nationally in these health outcomes. The program emphasizes reducing disparities among vulnerable populations, including racial minorities, low-income groups, older adults, and those with disabilities, by implementing evidence-based strategies focused on preventing and managing hypertension, high cholesterol, and other risk factors. The overarching goal is to improve health equity and outcomes through targeted interventions that consider social determinants of health. A key component of this effort involves establishing or expanding a statewide Cardiovascular Health Learning Collaborative (LC), which will serve as a backbone organization to foster collaboration among diverse partners such as public health agencies, healthcare providers, community organizations, and other stakeholders. The LC will facilitate communication, coordinate efforts, and leverage resources to implement culturally informed programs targeting populations with the highest prevalence of CVD and related conditions. The initiative emphasizes addressing social determinants of health, reducing disparities, and promoting health equity by focusing on communities disproportionately impacted by cardiovascular risk factors. The contract process for this project is open, with a response deadline of March 26, 2024, and aims to strengthen statewide efforts to improve cardiovascular health outcomes through coordinated, evidence-informed approaches.

Full Description

The Cardiovascular Health (CVH) Program in the Bureau of Health Improvement and Wellness at the Ohio Department of Health (ODH) received a cooperative agreement from the Centers for Disease Control and Prevention (CDC), DP2304: The National Cardiovascular Health Program through June 2028. Ohio has a rich history of working in chronic diseases under various cooperative agreements from the Centers for Disease Control and Prevention, most recently Improving the Health of Americans Through Prevention and Management of Diabetes and Heart Disease and Stroke (DP-18-1815). The focus of the DP-18-1815 cooperative agreement was to support state investments in implementing and evaluating evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in high-burden populations/communities, contributing to improved health outcomes.Chronic diseases such as heart disease, stroke, diabetes, and many cancers are among the most common, costly, and preventable of all health problems in both the United States and Ohio. Chronic disease is associated with reduced quality of life, poor health outcomes, increased healthcare needs, and higher healthcare costs. In 2020, Ohio ranked among the worst in the country for cardiovascular deaths (42nd) and diabetes (39th). Ohio Vital Statistics data show that heart disease was the leading cause of death in Ohio in 2020, accounting for 30,528 deaths, and stroke was the 5th leading cause, accounting for 7,055 deaths.According to 2021 data from the Ohio Behavioral Risk Factor Surveillance System (BRFSS), Ohio adults have a higher prevalence of heart disease (7.6%), stroke (4.1%), and diabetes (11.8%), compared with the U.S. prevalence. More than 1 in 3 adult Ohioans have high blood pressure (35.6%), high cholesterol (34.4%), and obesity (36.5%).The burden of chronic disease disproportionately impacts certain populations in Ohio. The estimated prevalence of many chronic diseases (heart disease, stroke, diabetes, cancer, chronic obstructive pulmonary disease, asthma, and arthritis) are all significantly higher among populations with the lowest income and lowest education. Other groups of Ohio adults with a significantly higher prevalence of heart disease include those with disabilities (15.7%), veterans (15.0%), and those aged 65 and older (18.9%). In addition, adults aged 65 and older and adults with disabilities have a disproportionally high prevalence of stroke (8.7% and 9.1%, respectively).Chronic disease mortality rates by race/ethnicity indicate that Black Ohioans have higher death rates of heart disease, stroke, and diabetes, compared with other racial groups. For example, the heart disease death rate was 23.9% higher, and the stroke death rate was 31.8% higher, among Black Ohioans, compared with white Ohioans in 2020. There are also marked geographic disparities in Ohio. In 2020, the heart disease death rate in the 32-county Appalachian region of Ohio was 13% higher, compared with non-Appalachia Ohio. The goal of the ODH CVH Program is to support implementation and evaluation of evidence-based and evidence-informed strategies to prevent and manage cardiovascular disease (CVD) in populations disproportionately at risk of CVD, exacerbated by health inequities and disparities, and social determinants of health. Specific emphasis will be placed on the prevention and control of hypertension and high cholesterol.  The objective of this project is to contract with an organization to establish or expand a statewide Cardiovascular Health (CVH) Learning Collaborative (LC) of diverse partners to address CVH, including hypertension, high cholesterol, risks for CVD, and the social determinants of health that impact CVH. The selected contractor will serve as the backbone organization and will work in partnership with ODH to recruit member organizations, coordinate and lead the LC, and conduct a landscape analysis of the current efforts happening in the state around cardiovascular disease prevention and management and the social determinants of health that impact it.For the purpose of this project, a LC is defined as a group of public health entities, health systems, health care providers, and community leaders and their partners with experience working to address and implement evidence-based or evidence-informed practices for CVD prevention, detection, control, and management. The LC will facilitate communication and the exchange of ideas and leverage technical and financial resources to support programs to improve cardiovascular health outcomes for all persons, but specifically those with or at highest risk of poor cardiovascular health outcomes.LC membership may include, but is not limited to: public health entities; housing, commerce, and transportation agencies; health systems; health care providers; local health departments; local or regional coalitions/alliances focused on cardiovascular health, primary care associations; clinical quality improvement organizations; health information exchanges; public and private payers; pharmacists; mental and behavioral health professionals; community-based health care professionals; community organizations; safety net providers; area agencies on aging; colleges and  universities; etc. By engaging these partners, the CVH LC aims to collaborate with and build upon the existing work happening throughout the state to improve cardiovascular health. The LC is expected to:• Prioritize populations and communities with the highest prevalence of CVD, with a focus on advancing health equity for individuals with hypertension and high cholesterol. • Serve populations and communities affected disproportionately by hypertension, high cholesterol, or stroke due to social determinants of health and other factors such as limited access to care, inadequate or poor quality of care, or economic instability. • Achieve optimal health outcomes for priority populations through culturally informed programs and services that use focused strategies to advance health equity goals that are mindful of the social determinants of health.
