Great Onyx JCC Medical Services

This contract requires comprehensive on-site medical services at the Great Onyx Job Corps Center in Mammoth Cave, Kentucky, for all students residing at the facility. The scope includes initial physical examinations, development of medical treatment plans, ongoing diagnosis and treatment, prescription issuance, immunizations, health monitoring, referral coordination, prevention planning, and educational outreach to staff, all in strict accordance with Title 20 CFR Part 684, HIPAA, OSHA, and applicable federal, state, and local regulations. Services must be delivered weekly by a licensed Physician or Nurse Practitioner/Physician Assistant on a schedule agreed upon with center personnel, typically one or two days per week between 8 a.m. and 5 p.m., Monday through Friday, with no services performed on designated federal holidays. All student health records and generated reports are the exclusive property of the U.S. Government, and the contractor is obligated to maintain the confidentiality and security of these records at all times. The contractor must ensure that all personnel are properly licensed, carry malpractice and general liability insurance, and undergo criminal background checks and drug testing as required by the Contracting Officer to protect student health and safety; failure to meet these standards may result in immediate contract termination. The solicitation is a total small business set-aside under NAICS code 621111, open only to responsible small business concerns, with no award to be made until funds become available. Any quotation must include pricing and complete documentation for proposed personnel, including resumes, proof of insurance, and valid state licenses, or risk being deemed non-responsive. Invoices must be submitted monthly through the INVOICING PROCESSING PLATFORM between the first and fifteenth of the following month, detailing only services rendered during that period and billed according to specified contract line item numbers. Key personnel must provide current contact information to the Center HWD or COR and coordinate all absences or substitutions with the Contracting Officer and center leadership 24 hours in advance, with all substitutes requiring prior approval from national and regional offices. The government furnishes all equipment and supplies needed to perform the work, and contractors are responsible for securing any space or materials used in accordance with center security policies. Travel is not authorized, and the government reserves the right to cancel this solicitation at any time without financial obligation to offerors.
