PERFORMANCE WORK STATEMENT (PWS) for Required Services TITLE: Statement of Conditions/Life Safety Assessment services to support the Sioux Falls VA Health Care System (SFVAHCS)
GENERAL OVERVIEW: Contractor shall update The Joint Commission Statement of Conditions and update life safety drawings for the Sioux Falls VA Health Care System in accordance with Joint Commission Hospital Physical Environment Document List and Review Tool, NFPA, and the most current VHA Joint Commission Clarification Document. Contractor shall determine NFPA 101-2024 (or later) Life Safety Code (LSC) deficiencies in accordance with NFPA 101, NFPA 99 and the most current Joint Commission Clarification Document and VA modified Document List and Review Tool. PLACE OF PERFORMANCE: Services will be performed at: Sioux Falls VA Health Care System 22nd Street, Sioux Falls, SD 57105
HOURS OF SERVICE: The Contractor shall perform the services as specified herein Monday through Friday between 8:00am 4:30pm, except national holidays, or when the government facility is closed due to local or national emergencies, administrative closings, or similar government directed closings. The Contractor is not required to provide services on the following national holidays, nor shall the Contractor be paid for these holidays:
PERIOD OF PERFORMANCE: This will be a Firm Fixed Price contract. There will be one (1) base period and up to four (4) option periods for this contract. Exercise of option periods will be made at Government s discretion based on continued need for services, Contractor performance, and availability of funds in a given fiscal year.
CONTRACTOR QUALIFICATIONS: Contractor shall provide the VA with proof of the following qualifications prior to contract award and upon request. Failure to submit any of the required information, statements, or certifications may result in rejection of the quote without further consideration for award or termination of the contract after the award.
All assigned surveyors must have documented experience with NFPA 101 Life Safety Code and NFPA 99 Health Care Facilities Code. Assigned surveyors will have at least five years experience performing Fire Life Safety Assessments and shall have conducted at least 3 hospital assessments, with demonstrated experience in evaluating complex healthcare facilities with respect to The Joint Commission accreditation standards.
Fire Protection Engineer shall be onsite to perform the assessments.
Fire Protection Engineer shall be able to accurately assess Healthcare Occupancy, Ambulatory Healthcare Occupancy, Business Occupancy and Special Purpose Industrial Occupancy.
Shall be able to provide AutoCAD drawings.
