ICDBG 2023 - $45,000.00 **after foundation** Federal LEAD-POSITIVE HVAC, ADA work-maybe Score - 62 Age - 68 (spouse's age) HBD - 1972 DFD - 7/19/1990 Deed in tenant and husband's name.
Special Instructions: Home is a 3-bed 1.5 bath, brick home approx. 1600 sq. ft. shingle roof. Slab on grade. Case Manager- Jeannine Davis 580 642 6384 Inspector- Corey James 580 317 7088 This will need to be sent to a lead base paint contractor. All prospective contractors are required to visit the project site before submitting a bid. Change orders due to conditions that could have been identified during the site visit will not be considered or approved.
Lead Base Paint: All work must meet or exceed the 2019 IRC. All applicable licenses will apply. All Lead Base Paint repair, removal, enclosing, or encapsulation must be completed by a Certified Lead Safe contractor. Per scope of work, if repairing, removal, enclosing or encapsulation is required, the Choctaw Nation Housing Authority will order a clearance test after job completion to verify lead has been removed, enclosed or encapsulated according to EPA/HUD standards and guidelines according to the scope of work submitted for bids. Payment will not be issued until home has been retested and cleared of all LBP. Contact the Housing Authority for a copy of the Lead Base Paint Report containing exact locations of LBP.
Electrical: Upgrade electrical to a 200-amp service (including incoming service if required). Install new breaker box and breakers (label breaker panel), new wire throughout, receptacles, switches, 2 exhaust fans, fixtures in/out, gfci's and ceiling fans, (6 in Home). Repair all damage due to electrical change outs, to include but not limited to drywall, paint, paneling, flooring, and ect.
Drywall: Remove and replace all damaged drywall throughout home. Remove attic fan in hallway. Install new ½” drywall, tape, bed, texture and paint interior. Lead base paint on tile in master bathroom. Seal off bathroom and remove tiles offsite and dispose of properly.
Plumbing: Master Bathroom. Install 2 Comfort height/ADA commode, wax ring, and supply and shut offs. Remove vanity and install a pedestal sink. Install new water and waste supply lines. New shut off valves. Install 3 ADA grab bars in bathroom.
Mechanical-HVAC: Remove and replace the existing HVAC unit. Install a new minimum 14.3 SEER2 rated energy efficient electric heat pump (or gas) unit. Install new copper line-set, appropriate filter drier, thermostat, and registers. Caulk all ceiling drops to drywall before air register installation. Seal the return air cavity with drywall for energy efficiency. Insulate the duct where needed. Replace the plenum if required for proper fitment of the new unit. If duct board is to be used for transition to plenum, all joints must be foil-taped, and an acrylic latex or mastic sealant is to be applied over foil-taped joints. Ductwork that is to be installed or replaced under the home should be insulated metal pipe; flex duct is not to be used underneath the home. The contractor will be responsible for the proper sizing of the new unit. The contractor must provide the AHRI certificate for the equipment to be installed with the invoice for payment. Verify that all electrical components servicing the HVAC system-including wiring, breakers, panels and fuses-are properly grounded and compliant with all applicable codes. Any non-compliant conditions shall be corrected by the contractor.
Doors: Remove and replace 1 entry door. Install 1 pre-hung six-panel insulated metal doors. Install all trim required for proper fit and finish. Install weatherstripping on exterior doors. Install new combination, keyed alike hardware, Kwikset or comparable. Caulk and seal inside and out. Paint to match new or existing finishes. Install 2 storm doors. Include all hardware. Remove and replace all interior doors, approx. 12. Install new pre-hung interior doors, trim and hardware (Kwikset or comparable). Stain or paint to match new or existing finishes.
Windows: Remove and replace all windows throughout the home. Install new insulated vinyl, low E, tilt sash windows. Repair any, and all damage due to window replacement, including paint, all trim, including window stools/sills if required, inside and out. Caulk and seal inside and out. Repair or replace any and all deteriorated framing as needed for proper repair. There are approximately 8 singles, 3 double windows in home. Contractor is responsible for exact window count and measurements.
Rough Carpentry: Cover all exposed exterior wood surfaces with PVC-coated coil metal (Fascia). Replace any deteriorated Fascia, approx. 40 lineal feet, and exterior trim needed before applying coil metal.
Special Construction: Install french drain on North East Corner of home. Go out approx. 10' East and come back south approx. 35' to drain out in front of home. (Water is flooding east door/wall into home. Install sod back on areas disturbed.
Flooring: Install new waterproof floating vinyl plank flooring with a minimum 3/16” thick flooring on add-on room, bedrooms and bathroom. Include transition strips. Pull toilet and install flooring up to the floor flange. Install all new trim required for proper fit and finish. Participant will choose color. (Flooring can be overlaid)
Special Equipment: The Contractor is to provide a storage container and move all the contents out of the house before working on the inside and back into the house after the job is completed.
Special Instructions: All work done must meet or exceed 2019 IRC. All applicable licenses apply. It is highly recommended contractor view job before biding. Contractor is responsible for all line locates and permits required Contractor will be responsible for cleanup and removal of all waste and repair yard back to mowing condition. Homeowner/Property Manager will select any/all colors used for the job. Contractor is responsible for all measurements, and count of materials needed.
